Yuma County FQHC makes accessing care easier for patients with improved call center efficiencies and online patient self-scheduling

 

Nashville, TN, August 22, 2019MyHealthDirect, the national leader in digital care coordination solutions, announced today that Sunset Community Health Center (Sunset) will leverage its platform to make access to primary and specialty care easier for its 35,000 patients. Sunset, a Federally Qualified Health Center (FQHC), operates out of 13 clinic sites throughout Yuma County in the extreme southwest corner of Arizona. Family health care site locations are strategically located in designated health professional shortage areas and/or medically underserved areas throughout Yuma County to better assist the patient population by providing them with a medical home.

With the MyHealthDirect platform, Sunset will improve scheduling within its centralized call center, where patients call in to book an appointment. While on the phone, patients will answer some simple questions, and those answers will guide the agent to the most appropriate provider and appointment time for that patient.

Sunset will also enable patient self-scheduling on their website. Following the same question and answer process, patients will be able to schedule an appointment online with the best-fit provider. The platform ensures bookings are accurate while also making the experience much more convenient for the patient because they can book any time, day or night, from any Internet connected device. Once booked, either by phone or online, patients receive a smart phone calendar reminder that includes instructions for their appointment.
