THE JOB WE DO FOR: Managed Care and Case Management

MHD gives health plans the ability to match and make appointments for their members, including their previously assigned Primary Care Providers.  Through member services, care managers, and nurse triage telephone lines, MHD can improve service through instantaneous scheduling of appointments. By referring and scheduling patients to appropriate primary care homes, MHD helps health plans proactively address issues such as treating primary care conditions in a high cost environment like the ED.  MHD’s solution, when in place in a network or community, also helps to improve continuity of care, providing for seamless appointments across multiple provider sites.
